If you are setting up a private clinic, hospital, nursing home, or diagnostic centre in Karnataka, KPME registration is not optional it is the law.
The Karnataka Private Medical Establishments (KPME) Act requires every private medical facility to obtain a licence certificate from the Directorate of Health Services (DHS), Karnataka, before commencing operations. Running a medical establishment without a valid KPME licence exposes you to penalties, forced closure, and legal proceedings from the state Health Department.
At SriMas Associate, we are experienced KPME consultants in Bangalore who have guided doctors, clinic owners, hospital administrators, and diagnostic centre operators through the entire KPME registration process from document readiness to final certificate issuance. Whether you need new KPME registration, online KPME registration renewal, or help resolving a lapsed licence, we handle it correctly, completely, and without unnecessary delay.
KPME stands for Karnataka Private Medical Establishments. The KPME Act originally passed in 2007 and amended in 2017 to include rate-fixing powers and patient grievance redressal brings all private medical establishments in Karnataka under direct government oversight, ensuring they maintain defined standards of infrastructure, staffing, safety, and patient care.
KPME registration is the mandatory licence that grants a private medical establishment the legal right to operate in Karnataka. It is issued by the Directorate of Health Services (DHS) through the official KPME portal (kpme.karnataka.gov.in) and must be displayed prominently at the facility premises along with the registration number, owner or manager name, system of medicine, and other mandated details.
The 2017 amendment also introduced colour-coded display board requirements:
Non-compliance with display norms, in addition to operating without registration, invites action during regular DHS enforcement drives.

The KPME online registration process is handled entirely through the official government portal at kpme.karnataka.gov.in. Here is the complete step-by-step process:
Ensure your clinic or hospital meets the infrastructure, equipment, staffing, and safety prerequisites under the KPME Act. Collect all required documents (see list above), have them scanned in the correct formats, and ensure all certifications are current and valid.
Visit kpme.karnataka.gov.in and click “Establishment Sign Up”. Enter your establishment name, address, ownership details, system of medicine, and category to create your portal account.
Log in to your account and select “New Establishment Registration”. Fill in the complete application with establishment name, address, category (clinic/hospital/lab), type of ownership, contact details, and system of medicine. Upload all required documents as scanned copies.
After filling the application, check the applicable fee using the “To Know the Fee” option. Make the online payment through the portal’s payment gateway. Download and save your payment receipt.
Review the filled application, digitally sign it (e-sign), and submit. You will receive an application reference number retain this for tracking.
The DHS will schedule an inspection of your premises. An authorised KPME committee will visit and verify compliance with infrastructure, safety, equipment, and staffing requirements. Address any deficiencies flagged during inspection promptly.
Upon satisfactory inspection, the registration certificate is generated and available for download through the portal. Display it prominently at your establishment as mandated.
KPME registration is not a one-time requirement. It must be renewed periodically as per the DHS schedule. Failure to renew on time can result in your establishment being treated as unregistered, making it vulnerable to raids, closure notices, and fines.
The documents required for KPME registration renewal largely mirror those for initial registration, with the following additions:
Any changes to your establishment since the last registration including change of address, addition of beds, expansion of services, change of ownership, or change of the key medical officer must be declared and applied for separately as a Modification on the portal.
Do not wait until the last day. Processing times vary and portal delays or document queries can push your renewal past the deadline. SriMas Associate recommends initiating renewal at least 60 days before the expiry date.
We are a Bangalore-based compliance consultancy with deep, hands-on experience in government registrations across healthcare, property, and trade licence domains. Doctors and clinic owners across Bangalore choose us because we eliminate the two biggest problems with KPME registration: wasted time and application errors.
Infrastructure Standards Premises must maintain the standards of construction, space allocation, sanitation, ventilation, and emergency infrastructure specified for your category of establishment.
Qualified and Registered Staff Every medical practitioner, nurse, and technician working at the facility must hold valid qualifications and current registration with the relevant statutory council (KMC, Karnataka Nursing Council, etc.).
Bio-Medical Waste Management Strict compliance with CPCB Bio-Medical Waste Management Rules 2016 is mandatory. Your tie-up with an authorised bio-medical waste handler must remain current and documented.
Patient Rights and Ethics The amended KPME Act (2017) specifically protects patient rights including the right to transparent treatment rates, right to emergency care, and right to file grievances. Facilities must maintain prescribed records and be prepared for DHS audits.
Display Requirements Post-registration, you are legally required to display a colour-coded board at the entry with your KPME registration number, establishment name, owner/manager details, and system of medicine. Failure to display is a compliance violation.
Operating without KPME registration or with a lapsed, unrenewed registration is treated as an illegal act under Karnataka law. Recent enforcement drives by the DHS have led to raids and forced closures of unregistered facilities across the state.
KPME registration is the mandatory licence required for all private medical establishments operating in Karnataka, issued by the Directorate of Health Services under the Karnataka Private Medical Establishments Act. It certifies that your facility meets the state’s healthcare infrastructure, safety, and staffing standards, and is the legal permission to operate as a private healthcare facility.
The key documents required for KPME registration are: proof of ownership or lease, building plan, fire safety clearance, bio-medical waste disposal certificate, medical qualification certificates, Medical Council registration, NOC from local authority, staff details, and equipment list. See the complete checklist in our documents section above.
KPME clinic registration fees depend on your establishment’s category and sub-category. A single-doctor consultation clinic typically falls in the ₹500–₹1,000 range, while multi-specialty or hospital facilities attract higher fees. The exact fee can be confirmed via the KPME portal or by contacting SriMas Associate for a free fee assessment.

KPME renewal is done online through the same portal. Log in, navigate to the renewal section, upload updated documents, pay the renewal fee, and submit. SriMas Associate recommends starting renewal at least 60 days before expiry to avoid a lapsed registration.
